If you add a set of stairs on the second floor it should automatically add the third floor.
They are unable to access the second floor.
Choose your stairs again and place them by holding the stairs close to the third floor.
You need 2 squares of space free from the frieze to the upper floor for this to work.
Adding stairs can be as simple as placing a few steps from the front door down to the ground or it can be that tricky feat of getting a set of stairs down through your foundation to the basement below.
In your new build you are putting your stairs right up against the wall on the first level which is fine but in order for them to be usable to get to the second level they need that same space flooring at the top.
In your original build you have enough space between the wall and the stairs.
More worryingly sims 3 automatically places floor tiles by stairs and those seem to be just not there.
